Firstly, this will be a one off, you will not be able to change it again. So please be very sure you want this change and have the name exactly as you want it (including the case of all letters other than the first). I will copy the new name from my talk page, and if it's wrong you are stuck with it.
Generally, I don't want to over-use this: it's confusing to people looking for contributors, it's not good in terms of the attribution required by the license, and it's heavy on the servers. It's easier to just make a new account and redirect your old page to your new one. Or why not just change your sig to another version? So please think about it, and let me know if you are certain you want to go ahead.
